Fallodon Way Medical Centre works with
professionals from other organisations to form a
broad multidisciplinary team.
Community Matron:
Lorraine Little is a highly experience nurse
who is employed directly by the Primary Care
Trust. Her prinicipal role is the community
management of elderly patients with complex
medical problems, working to enable them to
remain at home wherever possible.
Midwifery:
The midwives are based at John Milton Clinic
(tel : 950 0844), but see antenatal patients in
the surgery after their initial booking
appointments. Mary Davey has been our lead
midwife for many years
District Nursing Team:
The district nurses are also based at John
Milton Clinic (tel : 950 0844). A dedicated team
with a broad skill mix, they offer nursing
services to all truly housebound patients
Health Visitors:
The health visiting team is also based at
John Milton Clinic (tel : 950 0844), and
comprises Health Visitors and Nursery Nurses,
working to meet the needs of preschool children
and their parents. They have a regular clinic at
the Reedley Road Baptist Church on Tuesday
1.30-2.15. They are the first port of call for
advice on feeding and sleep issues and can be
contacted directly.
Dementia Support Officer:
Jake Halls is employed by the Dementia Care
Trust, and works with local practices to support
patients with dementia and provide advice to
their carers. He can guide patients to a number
of sources of practical support. If you feel
that you or someone you know would benefit from
his input, please inform reception who can put
you in touch with him directly.
Pharmacists:
The practice has 2 visiting pharmacists –
Alison Alvey and Patrick Durant. They are both
highly experienced and are able to offer
detailed advice on all aspects of medication
use, including what your tablets are for and how
to best to take them. They are both available
for face to face consultations (on Tuesday and
Wednesday mornings respectively). If you have
any questions or concerns about your medication
you can book an appointment with them directly
by calling reception. We also encourage all
patients who have recently been in hospital, and
whose medication has changed as a result, to see
the pharmacists to discuss the changes
