It is very important for us to know if there
are any changes in name, address and all
telephone numbers. We would like this
information in writing - a form is available
from reception.
Please ensure all relevant family changes are
passed on.
Mobile telephone numbers are very helpful in
contacting patients. In addition, they enable
access to our text
messaging system. This is used to remind
patients of appointments if they have been
booked forty eight hours in advance, with an
option to cancel and also to promote health
messaging. Please let us know if you would like
to participate in this service.
Some patients have a carer and we would be
grateful if you could let us know your carers
name, address and contact telephone number.
Also if you are a carer for a patient
registered at this practice do let us have these
