It is very important to let us know if you have changed your name, address or telephone numbers. This must be in writing- a form is available here, at reception or you can write us a letter.
We will need to see evidence confirming a change of name, e.g. marriage certificate or deed poll. If you have moved house within our practice boundary then we will need to see paperwork which confirms your new address, such as a utility bill or bank statement.
Please ensure you include all relevant family members under the age of 16. Patients aged 16 or over must advise us separately and provide their own documentary evidence.
Mobile telephone numbers are very helpful in contacting patients. In addition, they enable access to our text messaging system. This is used to remind patients of appointments if they have been booked forty eight hours in advance, with an option to cancel and also to promote health messaging. Please let us know if you would like to participate in this service.
Some patients have a carer and we would be grateful if you could let us know your carers name, address and contact telephone number.
