New Patient Registration
All patients who wish to join this practice, and who live within the practice area and are not registered with another GP in the area, should apply in person, bringing:-
- photographic proof of identity (e.g. passport/driving licence) and
- proof of address (e.g. utility bill/bank statement/tenancy agreement)
- registration forms and new patient questionnaires
Registration forms and new patient questionnaires are available from the reception desk at the surgery
If you are on repeat medications, you will need to see a doctor before requesting your next prescription.
Practice Area
The practice area covers Henleaze, Westbury Park, Westbury-On-Trym, Stoke Bishop and some parts of Redland.
The boundaries are: Sea Mills Lane, Bell Barn road, Canford Lane, Doncaster Road, Kingsholm Road, Kendon Drive, Wellington Hill West, Gloucester Road, Lower Redland Road, Rockleaze and Portway.

In common with all practices, our list is open only to patients living within the practice area. If you move out of the area you are kindly asked to register with a new practice that covers that address.
