Patient Participation Group
YOUR SURGERY
NEEDS YOU
In order to improve the service we deliver,
we would like to set up a Patient Participation
Group to occasionally ask for views on a range
of subjects relating to the Surgery. We
would also like to be able to email newsletters,
and any other useful information.
Initially we will try this new scheme out for 1
year.
If you would like to be involved, please ask
at reception for a form or contact the Practice
Manager (Debbie Cook). Any views or
comments you give will be anonymised, and will
have no effect on your medical treatment or care
provided by Fallodon Way Medical Centre.

Dear Patient,
We would like to know how we can improve our
service to you and what you think about the
services we provide.
To help us with this, we are setting up a
Patient Participation Group so that you can have
your say. We will ask the members of this
representative group some questions from time to
time, such as what you think about our opening
times or the quality of the care or service you
received. We would like to set up a 'virtual'
Patient Participation Group and will contact you
via email. Our aim is to keep surveys etc
succinct so it shouldn’t take too much of your
time.
We also have a Patient Reference Group which
will meet here at the surgery quarterly to
discuss the results of surveys etc. If you
would like to be part of this group please
contact the practice.
We aim to gather as many patients as possible
from as broad a spectrum as possible to get a
truly representative sample. We need young
people, workers, retirees, people with long term
conditions and people from non-British ethnic
groups.

occasionally by email - or would like to join
our Patient Reference group please either click
