Medical Exemption

People suffering from one of the following conditions can get a medical exemption certificate. Ask for an application form FP92A from reception.

  • a permanent fistula (for example caecostomy, colostomy, laryngostomy or ileostomy) requiring continuous surgical dressing or requiring an appliance.
  • a form of hypoadrenalism (for example Addison's disease) for which specific substitution therapy is essential.
  • diabetes insipidus or other forms of hypopituitarism.
  • diabetes mellitus, except where treatment is by diet alone.
  • hypoparathyroidism.
  • myasthenia gravis.
  • myxoedema (that is, hypothyroidism requiring thyroid hormone replacement).
  • epilepsy requiring continuous anticonvulsive therapy.
  • a continuing physical disability which means the person cannot go out without the help of another person.  Temporary disabilities do not count even if they last for several months.
  • In April 2009 medical exemption became available for patients who are undergoing treatment for cancer, including the effects of cancer, or the effects of current or previous cancer treatment.

Pre Payment Certificates

A pre payment certificate will save money for anyone who needs 4 or more items in 3 months, or more than 14 items in 12 months. You can buy a pre payment certificate to cover a 3 month or 12 month period. To help spread the cost, you can now choose to pay for the 12 month pre payment certificate by Direct Debit monthly instalments. Ask for Form FP95, ring 0845 850 0030, visit https://www.ppa.org.uk/ppa/ppcdd/patient.do or visit a pharmacy that is registered to sell pre payment certificates. Our closest pharmacy that sells them is Boots, 55 Henleaze Rd, Bristol, BS9 4JT.
