How to Register with the
practice

All patients who wish to join this practice,
and who live within the practice area and are
not registered with another GP in the area,
should apply in person, bringing:-
- photographic proof of identity (e.g.
passport/driving licence) and
- proof of address (e.g. utility bill/bank
statement/tenancy agreement)
- registration forms and new patient
questionnaires

All newly registered patients must have a
‘New Patient Health Check’ with
a Health Care Assistant or Practice Nurse.
We consider this an essential opportunity for us
to review your current health and to identify
and document health problems prior to your
records being received from your previous
doctor. Please bring a list of your current
medication to the appointment.
If you are on repeat medications you
will need to see a doctor before requesting your
next issue.
Temporary Residents
Patients can be seen at surgery as temporary
residents, for urgent problems only, if they are
staying within our practice area, staying with
one of our patients and here for less than
6months.
Temporary patients should complete a
temporary Services Form (GMS3/99) prior to
seeing the doctor, available at reception.
Please arrive in good time for your appointment
to allow enough time to complete the form. The
home address on the form should show where they
actually live and the address in this area as
the temporary one, even though this maybe the
family address.
When a temporary resident stay is completed,
data is sent back to the patient’s registered
doctor to maintain the patient’s records.
Overseas Patients
Patients visiting from abroad who do not
qualify for NHS treatment can be seen as private
patients and will be charged as per
Non NHS Fees.
