How to request a repeat
prescription

want a repeat prescription please follow
guidelines below REMEMBER ALL WRITTEN
REQUESTS NEED 48HRS TO PROCESS

If you have already registered for the online
prescription service

New to the practice:
If you are newly registered at the practice
and are on regular repeat prescriptions please
make an appointment with a doctor before
requesting a repeat, so that the doctor can
authorise the medication and add to your medical
notes.
Making a request:
- Requests can be made in writing
- Electronically via Emis Access
We do NOT accept requests
via the fax or telephone.
Requests in writing: Requires 48hrs to
process
- Please use the counterfoil provided with
your last prescription.
- Mark clearly with a tick all the items
that you need
- If you do not have a computer slip,
write out a clear list of what you need
including dose and quantity, or request a
counterfoil from reception.
These requests can be handed in to reception
or posted to the surgery

follow instructions. Remember this is only
for ROUTINE REPEAT MEDICATION ONLY. Queries
must be received in writing. MESSAGES
in the box will NOT be actioned.

Collection of prescriptions:
- Personally or by nominated person from
reception
- You can have your prescription sent
direct to the chemist of your choice:
- Boots Henleaze Road / Southmead Road
- Lloyds Stoke Lane and W-O-T
- Northview
- Kellaway Pharmacy
- Pharmacy Plus
- Buxton and Grant
For this service you need to talk to chemist
of choice and obtain agreement as to the service
available I.e. collection, collection and
delivery and so on. Once agreed, please indicate
clearly with your next request that you wish
your script sent to this chemist. All repeat
prescriptions will then automatically go to this
chemist until the practice is notified
otherwise.
