For absences of 5 or less working
days - employers are responsible for
providing and completion of self
certification forms for members of their
staff.
The doctor will only give a sick note
if you are going to be absent from work
for more than five working days.
If your Employer requests a doctor’s
sick note for absences of 5 working days
or less there is a fee payable, as this
comes under the heading of Non NHS work.
