We hope this information will ease your registration and help you understand the way we do things.

For families, you may wish to take the registration forms away to complete, one set for each person. One member of the family should bring back the completed forms to the Surgery, showing some photo ID and proof of address (which could be the letter you received from the CCG advising you to re-register at a new practice). We will not need to see ID for the other family members at this stage.
In view of the numbers involved, it could take up to 3 weeks for you to be registered after we have received your completed forms and in the meantime you will remain patients of Bishopston Medical Practice. We shall send you a text message to confirm your registration is complete, so be sure to give us your mobile number.
If you have regular repeat medications, it has been agreed that Bishopston Medical Practice patients transferring to Fallodon Way Medical Centre can be issued with three months medication before they leave. Please ask the Bishopston team to supply these before you leave their care.
Our pharmacist will review any medications you are taking and may invite you to attend an appointment with a nurse, the pharmacist or a doctor.
If you have Long Term Conditions that require an annual review, we shall call you in for yours, unless you have already had one at Bishopston Medical Practice since 1st April 2019. From then on, you will be invited to attend your annual review in the month of your birthday.
