Online Access for Young People Aged 13+

When a child registered at our practice turns 13 years old, their online access to medical records changes in line with NHS guidance on confidentiality.
From age 13, young people may be offered their own online access to services such as:
- Booking appointments
- Ordering repeat prescriptions
- Viewing parts of their medical record
For confidentiality reasons, parent or guardian proxy access may be removed or limited when a patient turns 13.
What this means for parents and guardians
- Existing proxy access may stop automatically when your child turns 13.
- If continued proxy access is appropriate, it may still be granted with the young person’s consent and after review by the practice.
- The young person can also choose to manage their account independently
What you need to do
- On or near your young persons 13th birthday we will post to them a 13 year old proxy form
- On receipt of this we need your child to complete the consent part of the form to either grant proxy access, and to whom, or to confirm no parental/guardian access be given and as such confirm the young persons preferred contact details
If your child is 13 or over and you would like to discuss access:
- Please contact reception, or
- Speak to a member of the practice team.
We will be happy to explain the options available and support both parents and young people in managing their online access safely.

Notice for Patients Turning 16
When you turn 16, you are legally entitled to manage your own healthcare and make decisions about your medical treatment. At this stage, we will usually communicate directly with you regarding your care.
Please be aware that your medical record may still contain contact details for your parent(s) or guardian(s), which were added while you were under 16. These details are not automatically removed when you turn 16.
If you would like us to update or remove any parent or guardian contact information from your record, or if you would prefer that we only contact you directly, please inform a member of our team and we will be happy to review and update your contact preferences.
