Pregnancy Self Referral
Use Badger Notes to Self Refer your Pregnancy

Badger Notes is an NHS service providing a single point of access for all maternity referrals in the Bristol, North Somerset and South Gloucestershire area.
The aim is to make sure you have access to the right care and the correct maternity servies from the minute you find out you are pregnant.
Your referral will be triaged based on the information you provide and the hospital will contact you to start your antenatal care.
Use the link below if you are pregnant and wish to refer for antenatal maternity care at either North Bristol NHS Trust NBT (Southmead or Cossham) or University Hospitals Bristol and Weston NHS Foundation Trust UHBW (St Michael's or Weston General Hospital).
Once on the website, you will complete a self-referral form and a member of the team will be in touch to book your first appointment with the community midwife.
The form should take approximately 10 minutes to complete.
If you need help completing this form, you can contact the booking team Monday to Friday 8am – 4pm on 0117 414 6743 (North Bristol NHS Trust) or 0117 342 0850 (University Hospitals Bristol and Weston NHS Foundation Trust).
In the meantime if you are worried about your pregnancy, please tell your GP straight away.
If it’s an urgent issue or outside of your GP’s hours, consider contacting NHS 111 or visiting your nearest maternity unit.
