How to Request a Home Visit
The home visit service is for our housebound patients who are too ill or infirm to travel.
If a home visit is required, please phone before 12noon giving contact details, current address (and home address if different) and the nature of the problem to help the doctor assess and plan appropriately.
Often, a member of the practice care co-ordinator team will ring first to further assess the problem.
Home visits normally take place after morning surgery or when time permits.
